Dzeko, 32, enjoyed another impressive campaign for Roma - perhaps highlighted by his eight Champions League goals, a crucial factor in the club's run to the semi-finals of the competition.
Dzeko beat off stiff competition from goalkeeper Alisson Becker to clinch the fan vote, with club captain Daniele De Rossi rounding out the top three.
Aleksandar Kolarov, Federico Fazio and Kostas Manolas were the other players shortlisted for the prize - which was won last year by Radja Nainggolan.
